Medical imaging for eye disease diagnosis and treatment

Neuroophthalmologic lesions often cause blindness and neurological damage, causing major social health problems. Two teams from the Capital Medical University and the Institute of Automation of the Chinese Academy of Sciences (hereinafter referred to as the Institute of Automation) started their research and participated in the project “Creation and Promotion of Imaging Systems for Neuro-Ophthalmology”.

In the 2017 Awards for Excellence in Scientific Research Awards (Science and Technology) announced by the Ministry of Education, the above projects won the first prize of the Science and Technology Progress Award of the Higher Education Institution.

He Huiguang, a project participant and researcher at the Institute of Automation, said in an interview with the reporter of the Chinese Journal of Science and Technology: "The automation team is the main technical supporter of the project, and together with clinicians, is committed to the innovation of specific imaging technology in neuro-ophthalmology, and has achieved a series of Results."

Meet the technology star

In 2007, He Huiguang was funded by the Beijing Science and Technology New Star Program. At the meeting of the Science and Technology New Star, He Huiguang met Dr. Ai Likun from Tongren Hospital. The two sides began to work together to study the changes in the structure and function of the brain by amblyopia.

"At that time, because of the need to conduct research from the perspective of neuroimaging, I met Professor Wang Zhenchang, the deputy director of the Tongren Hospital Imaging Center, and the deputy director, Professor Xianjun (also a newcomer in science and technology)." He Huiguang recalled that we have started from here. Cooperation for more than 10 years.

“Tongren Hospital is famous for ophthalmology. Our team is interested in visual research. The cooperation is the first to choose the research of neuro-ophthalmology.” He Huiguang said, “From the perspective of automation, we are studying medical image processing and analysis methods, but The validation of the method requires hospital data support."

The cooperation between He Huiguang team and Tongren Hospital team not only harvested imaging experts, but also acquired experts in clinical ophthalmology. “Image experts provide us with image data, ophthalmologists provide clinical data and ophthalmology knowledge, and we are responsible for researching algorithms. The three parties complement each other and form a close-knit team.”

Neuro-ophthalmology is an emerging interdisciplinary subject closely related to ophthalmology and neurology. "Since the limitations of examination methods, neuro-ophthalmology diagnosis is difficult, it is difficult to achieve precise treatment." He Huiguang explained, "The medical community lacks awareness of the whole brain changes of common neuro-ophthalmic diseases and their mechanisms, leading to slow development of the discipline."

In response to the above problems, the "Neurology and Ophthalmology Imaging System Creation and Extension" project team has developed a clinical-image large database of neuro-ophthalmology diseases after 20 years of research, starting with solving key scientific problems and based on new theories, new technologies, and new strategies. Gradually created the neuro-ophthalmology imaging system in China and built an imaging research platform for neuro-ophthalmic diseases.

It is reported that the main accomplishers of the project include Wang Zhenchang, Xianjun, He Huiguang (third completion person), etc. Capital Medical University is the first completion unit, and the Automation Institute is the second completion unit.

Carry out clinical cooperation

"There is a close relationship between vision and brain function, but there has been a lack of powerful research methods to clarify the complex interaction between the two." He Huiguang pointed out that "MR (magnetic resonance) functional imaging is a new exploration of the interaction between vision and brain function. Methods, the project team used multimodal imaging technology to first propose glaucoma as a total visual path and multi-network nerve damage; for the first time, it was confirmed that congenital complex strabismus is only a sub-nuclear abnormality."

Due to the anatomical nature of the neuro-ophthalmology, although the main symptoms are only diplopia/strabismus, vision loss/blindness, the location and etiology of the lesions are complex and diverse. The project team is symptom-oriented and uses imaging methods to comprehensively evaluate the fine anatomy of the neuro-ophthalmology and related lesions in the region.

He Huiguang said: "This project has created a series of lesion-directed lesion detection strategies through a series of studies on 2550 cases, which resulted in a 76.5% detection of the cause of vision loss/blindness and a detection rate of 75.8 for the cause of diplopia/strabismus. %."

In addition, He Huiguang team innovated on the specific imaging technology of neuro-ophthalmology: the first MR (magnetic resonance) imaging scheme for the shape and volume of the lateral geniculate body; the multi-parameter quantitative analysis method for visual pathway was established to overcome the problem of occult lesion assessment.

The project team headed by Wang Zhenchang also established a large-scale clinical-image database of neuro-ophthalmic diseases with the largest sample size in China, and defined the neuro-ophthalmic disease spectrum, systematic data mining and research from clinical symptoms, imaging techniques, disease types and other dimensions. A vision loss/blindness, double vision/strabismus imaging evaluation system was gradually established.

Talent and results

The project also trained a group of talents, including 1 Beijing scholar, 2 Beijing leading talents, 7 Beijing Science and Technology Star, and 94 graduate students.

At present, the results of the project have published more than 30 SCI papers, and have been promoted and applied in more than 20 top three hospitals nationwide. The main innovations have been written in the head and neck CT developed by the China Radiology Branch in 2006 and 2007 respectively. The MR Scanning Specification Guide, the National College of Higher Education planning textbooks, and related professional books.

During the implementation of the project, it has hosted more than 30 national academic conferences and 66 national-level continuing education classes.
